Outreach and intensive case management services are provided to high risk patients
of the health center referred by their primary care provider or other providers
within the health center. Case managers work with the patient and their families
to focus on achieving an optimum outcome and appropriate utilization of health services.
The program goals are to reduce barriers and increase access to health care.
Health Care to the Homeless provides health care via mobile van services to the
homeless living along the Waianae Coast and Kalaeloa. A team of healthcare
providers delivers services to the homeless. The overall goal of the program is
to outreach via mobile health care services to unsheltered homeless individuals
and families, reduce barriers to health care by making available case management
services and other needed resources (screening, referrals to medical/mental health
care, food, clothing, household items, employment, identification documents, etc.)
to homeless individuals who might not otherwise receive assistance, with the ultimate
goal of appropriate, affordable, and available housing placement and self-sufficiency.
Case Management services are provided to high risk pregnant women who receive their
prenatal care at the WCCHC. The goal in providing supportive services is to enable
high risk pregnant women to receive quality prenatal care and achieve a healthy
birth outcome. These services have revealed the benefits to early identification
and tracking to intervene with those at highest risk for chronic conditions, such
as, diabetes, substance abuse, nutritional deficiencies, domestic violence, mental
health concerns, and other perinatal and pediatric risks. Free prenatal classes
are also offered to pregnant women and their partners in this community.
Substance using pregnant women receiving prenatal care at the WCCHC are offered
supportive services to engage them into substance treatment through BabySAFE (Substance
Abuse Free Environment). Supportive classes are also offered to these women
including pre-vocational (job readiness classes) and psycho-educational classes
(addressing health and safety, domestic violence, trauma, parenting, etc.). The
goals of this program are early identification, assessment and continued engagement
to enter substance treatment prenatally or postpartum.
The WCCHC provides transportation services to registered patients of the health
center, transporting them to and from their appointments with Health Center providers
and other providers referred to within the community. Transportation functions as
an extension of case management services, making every effort to ensure that patients
are on time and that they keep their appointments.
The Child Passenger Safety Seat Fitting Station provides certified technicians to
check the installation of car seats. The goal of this service is to increase the
proper use of child passenger restraints and seat belts on Oahu. The Health Center
offers car seat checks by appointment twice a week and through two community events
a year. Education on the proper use of car seats and seat belts is also provided
to various community groups and events/fairs.
Eligibility Coordinators assist patients with completing applications for medical
insurance and medication assistance as with the State Med-QUEST program, Medicaid,
Medicare Part D, State Pharmacy Assistance Program, Immigrant Health Initiative,
Sliding Fee Scale, etc. Selected staff also provide assistance with low income
housing applications, Supplemental Security Income (SSI), Financial and Food stamp
applications. Patient Assistance Services also provides assistance in completing
Advanced Health Care Directive, and coordinating interpreter services for clients
of the Health Center.
